Further to our most recent communication, could we please ask that parents/carers complete the attached NHS Test & Trace Consent form, please print & post or drop it into the academy from Monday 4 January 2021.

We will be posting a hard copy of this consent form to all parents/carers when we return to the academy on Monday 4 January 2021.
